 New Patient Health History Form

Your child should visit the dentist by his/her 1st birthday. You can make the first visit to the dentist enjoyable and positive. Your child should be informed of the visit and told that the dentist and their staff will explain all procedures and answer any questions. The less to-do concerning the visit, the better.

It is best if you refrain from using words around your child that might cause unnecessary fear, such as needle, pull, drill or hurt. The office makes a practice of using words that convey the same message, but are pleasant and non-frightening to the child.

If your child is over the age of 3, we ask that you allow them to accompany our staff through the dental experience. We are all highly experienced in helping children overcome anxiety. Separation anxiety is not uncommon in children, so please try not to be concerned if your child exhibits some negative behavior. This is normal and will soon diminish. Studies and experience have shown that most children over the age of 3 react more positively when permitted to experience the dental visit on their own and in an environment designed for children.

We strive to make each and every visit to our office a fun one!

For your convenience when visiting our office, please print the New Patient Health History Form, complete the information and bring it with you to your first visit.
(To print the form you will need Adobe Acrobat Reader.  If you do not have Adobe Acrobat, please

Hi!  Here I am with my mask on.  The reason I wear my mask is to stop germs
from my nose or mouth from getting on you. Below you
 can see some pictures of
one patient who is in the middle of doing some
of her dental work!

Open wide!
Look how clean Anna has brushed her teeth.
Do you know what the little pointer is?  That is
a "tooth-wiper" that will wipe away any germs
that might try to do bad things to Anna's beautiful teeth.

We can see all of Anna's beautiful teeth!  The little mirror is shining twinkles to light up her teeth.

Look at what Anna is wearing.  She is wearing a special blue suit.   She is wearing this for just a minute while she has some special pictures taken of her teeth.  The suit is very heavy.
